ADULT DAY HEALTH CARE

Medical, nursing, therapy, social, recreational and personal care services are offered at our Center during the day for adults who are frail, have dementia or other chronic conditions, or require help with activities of daily living. We monitor medications and symptoms associated with diabetes, heart disease, and high blood pressure, preventing unnecessary emergency room visits and hospitalizations. The social interaction, mental stimulation and recreation offered prevents a sense of isolation. By providing cargivers and participants the support they need, ADHC can prevent early placement into a nursing home.

Transportation is included.

HOW TO ENROLL

Anyone can make a referral to Adult Day Services (ADS). Families, health care professionals, neighbors or self referral are all welcome. You can do it by phone, email or fax.

The Enrollment Process and the Application packet will be explained fully and the Intake Coordinator will help you to complete the necessary paperwork.
