Enrollment Is Easy |
Each new participant and their family and/or family caregiver are invited to tour our facility prior to enrollment.
Our Intake Coordinator will call the new participant directly and/or participant's contact person. At that time, we present an overview of our services and programs, and discuss enrollment in the program. This can be done at NVHADS every weekday, except for Thursday.
The Enrollment Process includes an Application packet with History and Physical forms that are completed by the Primary Care Physician. A current TB test is required of all participants in our programs.

Evaluation Days |
Our evaluation process is comprised of 2 initial days of attendance during which the patient-participant meets with our Registered Nurse, Medical Social Worker, Physical Therapist, Occupational Therapist and Activities Coordinator.
As indicated, the Primary Care Physician gives further instruction about the participant's need for a consultation with our Dietician, Speech Therapist and/or Psychologist.
Each discipline performs a complete evaluation of the patient, which includes a thorough discussion with participant and their caregiver to receive input on special needs and concerns.

Individual Plan of Care (IPC) |
At the end of this process, our team develops an Individual Plan of Care (IPC) and schedules a team meeting.
During the Team Meeting, participant, family, and caregiver meet the Adult Day Services team. A review of the Individual Plan of Care takes place with a question and answer period for the family and patient.
The final IPC is approved by the ADS Medical Director, and it is updated by ongoing assessment and coordination of care to meet each participant's and caregiver's individual needs.
