Getting Enrolled |
Anyone can make a referral to Adult Day Services (ADS). Families, health care professionals, neighbors or self referral are all welcome. You can do it by phone, email or fax.
Our Intake Coordinator will call the patient and/or the contact person and invite them to come into the program for a tour of our facilities, explanation of our services, and to assess whether the applicant would be appropriate for ADS services.
The Enrollment Process and the Application packet will be explained fully and the Intake Coordinator will help the patient and/or caregiver to complete the necessary paperwork.
The patient or caregiver will be given History and Physical Forms that will need to be completed by the participant's physician. Please note that a T.B. test (completed within the last 12 months) is required as part of the History and Physical. We can provide this onsite on all weekdays, except for Thursdays. The Intake Coordinator can also work with applicants to get this paperwork back.
Once the Application packet (including the History and Physical Forms) is returned, the Intake Coordinator will call the patient and/or caregiver to schedule their Evaluation Days and Team Meeting.

What occurs during the Evaluation Days and Team Meeting? |
The Evaluation Days encompass three days of attending the program, during which the patient will meet with the Registered Nurse, Social Worker, Physical Therapist, Occupational Therapist, Activities Coordinator; and if indicated by a primary care physician, they may also meet with the Psychologist, Dietitian and/or Speech Therapist. A Home Environment Assessment will be conducted by the Social Worker as well.
Each one of the disciplines will do a complete evaluation of the patient, and with the patient and/or caregivers input, will develop an Individual Plan of Care.
The Team Meeting is in place, for the patient and/or caregiver to meet all of the staff at NVHADS and go over the Individual Plan of Care. This is also the first official day of enrollment at Adult Day Services.
